1 - Program Application
The first step in the process for students at the Urban Training Center is to apply for acceptance in the program. The form below should be completed by first time students (and needs to be completed only once). It serves to notify the Urban Training Center of your intention to take classes.
Upon completion of the form:
1) Send one copy, along with your registration fee ($35) to the District Treasurer at 631A Swedesford Rd, Frazer, PA 19355. You may pay your registration fee online, but be sure to use the "UTC & Ministerial Studies" line and include your name and "registration fee." You will still need to send the application form to the District Treasurer at the address above.
2) Send one copy to Dr. Chuck Gates at 3 Horseshoe Lane, Mullica Hill, NJ 08062.
Once this form is completed and copies and fees sent to the appropriate locations, you can begin to choose classes.
