Minister's License Forms
Please fill out the following forms and return them to the district secretary.
District Minister's License Application - This forms must be filled out by all those seeking a district license or the renewal of a district license.
Recommendation to the District Assembly - This forms must be filled out by the local pastor and board secretary to confirm the recommendation of the license candidate by the local church.
Local Minister's License Application - This form must be filled out for all those being granted a local license; it is to be completed by the local Church Board and submitted to the General Secretary via email or at 17001 Prairie Star Parkway, Lenexa, KS 66220. A second copy must be submitted to the district secretary.
