District Assembly Forms

The following forms should be downloaded, completed, and emailed to the district secretary.

Certificate of Election - This form contains the information for delegates elected from the local church to the District Assembly.

Church Directory - This form contains the local church information necessary to complete the District Journal.

Deceased Church Members - This form contains a record of local church members who died in the previous year.

Recommendation for District License - This form contains the local church board recommendation of a member for a District Minister's License.  One form should be completed for each District License candidate.

Reports to the District Assembly

Annual Pastor's Report - This report is required for all those serving as senior pastor.  This report is done online via the Church of the Nazarene website and requires a Nazarene Account login (the same login you use to pay WEF).  If you do not have a Nazarene Account, visit this website to sign up for one 

Minister's Report - This report is required for ALL active ordained and licensed ministers NOT serving as Senior Pastors.

Retired Minister's Report - This report is for all retired ministers.

Evangelist's Report - This report is for all evangelists.

Applications to the District Assembly

Application for District Minister's License - This form must be completed for every candidate for a District Minister's License.

Application for Ordination - This form must be completed for any individual seeking ordination or recognition of ordination credentials from another denomination.
